The diagram shows a graft (central third of the patellar tendon) secured at the femur with an interference screw. These screws can be resorbable or metal. The arrow shows the fixation of the osseous block in the femoral tunnel, here, using a metal screw. Sometimes anterior cruciate ligament reconstruction using a simple intra articular graft (patellar tendon, hamstring tendons, allografts or others) can be insufficient. An articular extra reinforcement appears the logical choice in very advanced anterior laxity to avoid all failures.
